An Overview of the Community Process

When a patient is referred to the care of the Community Team:

  1. A referral is sent to the Community Team.
  2. A member of the Community Team reviews the referral and if they accept it, adds it to Community with a Intervention.
    Note - Rejected referrals should not be added to Community.
  3. A Patient Care record is created.
  4. The Intervention is allocated to a team member, from either the Caseload Management or Patient Care screen.
  5. The Community Team record all care given to the patient in their Patient Care record. This continues until the patient is discharged from the Community Team and their Patient Care record is resolved.
