Scoring the Reported Edmonton Frail Scale (out of a score of 17):

  • 0-5 - Not Frail
  • 6-7 - Apparently Vulnerable
  • 8-9 - Mild Frailty
  • 10-11 - Moderate Frailty
  • 12-17 - Severe Frailty

Accessing the Edmonton Frail Scale

  1. From Consultation Manager, select Vision+ from the floating toolbar and select Calculators - Other - Edmonton Frail Scale:

  2. The Edmonton Frail Scale displays:

  3. Complete the questionnaire by selecting an option under each heading.

  4. On completion of all the questions the Clinical term, Event Date and Score display at the base of the screen.

  5. Select OK to save the score to the patient's record or OK & Attach to also record the questionnaire:

  7. To view an attachment, from patient's record, right click on the attachment entry and select View Attachment. The attachment displays as a PDF:
