Address Change out of practice area

If the change of address is out of practice area, the patient is removing from the practice list, ie is transferring out. See Patients who die or remove out of practice area.

Non-registration Links practices can use a Transferred Out Reason of Remove to new HA (see Permanent patient is transferring out (non-Reg Links)).

For Registration Links practices:

• If the practice know the patient's new address, they could amend it in Registration - Address, which will generate an Amendment Transaction (AMG) to the HA, indicating in the free text GP Notes that the GP no longer wishes to keep this patient on the list. Once a Deduction Transaction has been sent, capitation payments cease (see "Registration Links Summary").
• Alternatively, they can request a Deduction – see Deduction Request Transaction.
