History Entries which should be in an SDA

Most entries are imported successfully and filed in the correct structured data area (SDA).

However, some records may transfer without a valid clinical code, they appear as History entries with the Read code of 9bJ. Examples might be O/E Weight or Serum Cholesterol, another example might be a BP record with just a systolic reading, which appears as degraded with the BP reading in free text.

In these cases, you should add a new record (eg, Add-Weight, Add-Test Result, Add-BP) with the correct date and then delete the History record.

These degraded records should not be left and must be edited, as untouched they dol not appear in searches, QOF audits or graphs.

  1. From Consultation Manager , list the degraded records by double clicking Degraded GP2GP Records at the bottom of the Alerts.
  2. The degraded records are displayed under the Filtered tab, double click on the degraded line.
  3. The correct information from the original record displays in the Comments or Notes .
  4. A degraded record has the correct Read Description but the wrong Read code starting with 9bJ, if this degraded record is a history entry, simply edit the Read code to a suitable code. If this degraded record is not a history entry, but it normally appears under an SDA, such as weight, height, blood pressure:
    • Select Cancel.
    • Add a new record in the usual way ensuring you:
    • Change the Date of Recording from today's date to the date of the original record.
    • Remove the tick in In Practice.
    • Blank out the name of your GP.
    • Enter any value, such as a weight in kg.
    • Select OK to finish.
  5. Finally, delete the degraded record from the degraded record list, by right clicking on it and selecting Delete.
