Acute Mental Distress (NHS Wales)

This clinical template is designed to meet GP contract requirements in Wales for practices to partake in a Clinical Quality Improvement initiative, which are also part of the Quality Assurance and Improvement Framework (QAIF). The Acute Mental Distress (NHS Wales) clinical template assists clinicians to record essential and optional information to assess a patient's acute mental distress.

Accessing the Acute Mental Distress (NHS Wales) Clinical Template

To access the Acute Mental Distress (NHS Wales) clinical template:

  1. Open the patient in Consultation Manager.

  2. From the Vision+ floating toolbar , select Clinical Templates and then select Acute Mental Distress (NHS Wales):

  3. The Acute Mental Distress (NHS Wales) template displays:

    The template is split into three tabs:

    • History - Displays clinical information from the patient's record.

      See Viewing the History Tab for details.
    • Mental Distress - The main template tab, complete as required.

      Don't forget:

      • A red exclamation mark and black triangle display to indicate that the information required here is mandatory. You must enter data here before saving and closing the template.

      Note - Below the main template a resources section with useful links pertaining to this calculator displays.
    • Risk Assessment Information - Provides additional information, for example, risk factors for suicide and protective factors.

  4. Select Save & Close to save the data to the patient's record:
