Address Change - Change HA/Same GP (Non-Registration Links)
The patient has moved address, over the boundary to another TP/HB/CSA, but is staying with the same GP at the practice.
If the practice is non-Registration Links:.
- Select the patient.
- Select the Registration - Registration Details screen tab.
- If the patient is moving to another Health Authority, click on the Change TP button to display the Registration - TP Change screen.
- New Agency - Display a list of Health Authorities (Strategic Health Authorities) and select the new HA. The new Health Authority must be one of the practice HAs added in File Maintenance - Organisation with which you usually have registered patients.
- In the Reason box, and for England & Wales practices only, select New HA/HB/CSA / Same GP. The patient is moving address within the practice area to a new HA, but is staying registered with the same GP. This indicates the GP is willing to keep the patient. Note that in Scotland there is no New Health Board / Same GP deduction reason.
The Date Transferred defaults to today.
- Click on OK.
That would usually complete this action, but if the patient is changing status for any other reason:
- Click on the Change Status button, to display the Change of Registration Status screen.
- Display the list of registration status and select Transferred out.
- Enter a date when the patient removed from the list.
- Select a reason why they transferred out.
- Click on OK.
