Setting Consent for Sending Uptake Reports

You are required to give practice consent for Vaccine Uptake reports to be sent to ImmForm. Consent to send Vaccine Uptake reports to Immform is the selected default, therefore no action is required unless you want to stop vaccine uptake reports being submitted.

To set consent:

1. From the Vision front screen, select Management Tools - Control Panel - File Maintenance.
2. Select Usage .
3. The Vaccine Usage screen displays:

   

4. In the Consent to send Vaccine Uptake reports to Immform section either:
   • Tick to agree to submitting your Vaccine Uptake reports via ImmForm, this is the default.
   • Remove the tick to stop all Vaccine Uptake reports from being submitted.
5. Select OK to save.
