Cancellation Reason

After you have confirmed you wish to cancel an item/prescription, you must select a reason for cancellation:

• Prescribing error
• Clinical Contraindication
• Change to medication treatment regime
• Clinical grounds
• At the patient’s request
• At the pharmacy request

Additional text is optional. The reason for cancellation is not sent to the pharmacy.
