Lifestyle

Within Consultation Manager, the Lifestyle Summary form is accessible from:

  • Summary - Lifestyle - Right click on the type of data to add and select Add. Complete the form as required and select OK.
  • Navigation pane, right click on Lifestyle and select Summary. Right click on the type of data to add and select Add. Complete the form as required and select OK.
  • Navigation pane, right click on Lifestyle and select Data Grid or Management Plan. Select the row required to add data, complete the form as required and select OK.

Lifestyle data entries include:

  • Occupation - 0....00 Occupations displays. Double click in the READ Term window to reach the READ Dictionary selection screen. Click in Display by Hierarchy. Double click on the yellow folder 0... Occupations and make a selection from the READ dictionary.
  • Smoking - see Adding a Smoking Entry for details.
  • Passive Smoking - The default READ term is 13WF4 Passive smoking risk. You can choose a Cause of passive smoking: Parental Household, Work place, or Other. The box Passive Smoker can be checked if the patient is a passive smoker. The Read codes on the picklist include:
    • 13WF400 Passive smoking risk
    • 137I Passive smoker
    • 137U Not a passive smoker
  • Alcohol - seeAdding an Alcohol Entry for details
  • Exercise - The default READ term is 138.. 00 Exercise Grading, In Type of Exercise, in place of <None>, you may also choose Inactive, Moderate, Vigorous, Gentle.
  • Diet - The default READ term is 13A.. 00 Diet - patient initiated. In Eating Habits, as well as <None>, the options are Good, Moderate, Poor, Not Examined. In Type of diet, you can choose from None, Vegetarian or Vegan. We suggest that you could ignore the Type of diet entries, and instead, select a suitable READ code from Read Term, see Diet for details.
  • Contraception - see Adding a Contraception Entry for details.
  • Sleep Pattern - The default READ term is R005 [D]Sleep disturbances, but you can also select from its hierarchical codes, and E274 Non-organic sleep disorders and hierarchical codes.
  • Treatment Compliance - The default READ term is 8B3E Drug compliance good. You can also select from its hierarchical codes.
  • Residence - 13F, 13F1-13F7, 13F9-13Fz.- ie excludes 13F8 Hospital In patient but includes 13F81.00 Long stay hospital in patient. Residence records are stored under Lifestyle but can also be entered from the Summary - Elderly Assessment form.
  • Agency - The default is 13G.. 00 Domiciliary Services, but you can select from other options, such as 13G1. 00 District nurse attends, 13G6. 00 Home help. In Type ofAgency, choose from <None>, Meals on Wheels, District Nurse, Health Visitor, Social Services, Home help, Other.
